I am presenting myself for consideration for the Award for Excellence in Perioperative Nursing, to be given by AORN at the 2002 Congress session in Anaheim California. Strangely enough, you can nominate yourself, although it will take many contributing nominations to make a favorable impression on the awards committee. So I am asking all of you to help me.

AORN headquarters has told me that all sponsorship forms must accompany the application or they cannot be considered. So if you could return that page to me, I would be most grateful. As far as I am concerned, I do not win this award, we all do!
